Call for Nominations Pilipinas Rotaract MDIO and Rotary International Zone 10A invite Rotaractors in the country to submit nominations to the Citizenship, Sustainability and Social Responsibility (CSR) Youth Awards which aims to recognize outstanding STUDENTS, YOUNG PROFESSIONALS OR YOUNG ENTREPRENEURS who have initiated projects that demonstrate good citizenship, environmental sustainability or personal social responsibility.

Eligibility Requirements 1. 18-39 years old 2. Of good moral charater/ no derogatory record 3. At least two (2) years track record of volunteerism or implementing projects related to good citizenship, environmental sustainability or personal social responsibility 4. Must be able to showcase in the application one (1) project the nominee has initiated and managed which highlights good citizenship, environmental sustainability or personal social responsibility (supported by documentary evidences of outcome and impact to beneficiaries/communities)
Award Categories
CITIZENSHIP - Involves general volunteerism, involvement in public policy and activities, working with public service projects, interventions for indigenous people, persons with disabilities, senior citizens and other vulnerable social sectors, promotion of heritage, arts and culture
SUSTAINABILITY - Involvement in biodiversity, climate change, carbon footprint, alternative energy and fuels, organic farming, recycling, conservation and environmental education projects
SOCIAL RESPONSIBILITY - Involvement in corporate social responsibility projects that highlight integration of CSR into the business operating model including participation of the company's stakeholders in the CSR project

2 x 2 Formal Colored Photo (graduation photo / professional photo)
Curriculum Vitae
Certificate of Enrollment (for students), Certificate of Employment (for employees), or Papers citing proof of business ownership or incorporation (for entrepreneurs)
Two recommendation Letters from a mentor, professor, church or civic leader who can tell about your passion for the project to be showcased and its results to its beneficiaries
CSR Project Portfolio (Includes photos / documents / writeups etc. that best showcase your chosen project. Kindly add to your portfolio all assessment / evaluation reports to your projects and/or financial reports as applicable. You may include links to relevant websites, social media, and other online presence. Please upload in a consolidated PDF file)
Three (3) minute video answering the question: "Why do I deserve to win the CSR Youth Award?" (No specific style required for the video)
Certificate of Membership signed by your Rotaract Club President
Note: Incomplete submission of requirements may not be considered.

August 31, 2020 - Deadline of submission of requirements to the Pilipinas Rotaract MDIO
September 1-7, 2020 - Pilipinas Rotaract MDIO and Rotary International Zone 10A Screening
September 8, 2020 - Endorsement of Rotaract Semi-Finalists to the CSR Youth Awards 2020 Screening Committee
September 30, 2020 - CSR Youth Awards 2020 Virtual Awards Ceremony
